French Language Services Health Promoter

Purpose:
Rexdale Community Health Centre (RCHC) is a non-profit, multi-service, community-based organization that serves the communities of Rexdale and North-West Etobicoke.
Operating out of five locations, RCHC offers core services that include primary health
care, health promotion, community development programs, Pathways to Education, and
Diabetes Education Centre.

The French Language Program Services Health Promotor will be responsible for
developing, implementing and facilitating programs to increase awareness of RCHC
programs and services, provide educational support about health issues that affect the
community and support identification of Rexdale area community needs and priorities.
Responsibilities:
 Develop and implement community-based health promotion and wellness programs in
French, in conjunction with other staff and the Coalition for Healthy Francophone
Communities in Rexdale that are responsive to needs in accordance with the program
work plan and strategic directions of the Centre.
 Deliver linguistically and culturally appropriate health promotion activities, including
workshops on lifestyle changes, physical activity, and healthy eating in French.
 Act as a resource for Francophone community groups by facilitating workshops,
seminars, information sessions or groups; providing consultation, referrals and
information; and advocating on behalf of or for groups in the community.
 Build awareness of the RCHC mandate and the range of community education services by facilitating presentations, workshops, and other public education events.
 Mobilize the existing Francophone human and material resources in Rexdale to enhance
the capacity of the community to affect social change by developing self-help and social
support networks.
 Develop and maintain project schedules and timelines for events.
 Engage with individuals, community groups and organizations through various
methods to meet their diverse needs and preferences.
 Maintain familiarity with services and programs in the community and share this
information organizationally as appropriate.
 Conduct outreach, develop community engagement plans, organize community
engagement events, execute special events and keep pre and post event details.
 Develop and maintain information on Francophone resources for the Centre and
informational/ educational displays on selected health topics as required.
 Contribute to building excellence and quality of programs and services, including
developing best practice models for engaging and working with Francophone
communities of racialized backgrounds.
 Act as a resource and liaison to the Central West LHIN French Language Services
Coordinator.
 Evaluate programs to ensure that objectives are met and make changes to the day-to-day running of the projects based on community input.
 Assist and advise the Centre on developing and maintaining resources and information
in French for distribution to the community, both printed materials and electronic
information for the website and social media.
 Submit program statistics and reports on a timely basis.
 Identify and implement improvement strategies and best practices to support the community
engagement process.
 Maintain records and statistical data in a timely fashion to produce reports and prepare the final community engagement report.
 Coordinates with purchase of service and contract staff.
 Perform other duties and committee work as assigned.
 Adhere to the Centre’s policies and procedures, including confidentiality, AODA, and
health and safety policies.
Qualifications:
 Post-secondary education in health, social work, social science or related discipline.
 At least 3 years work experience in a community development position and/or the
community services sector working in a culturally and racially diverse community based
setting.
 Proficiency in French, both written and verbal.
 Experience with EMR charting systems such as PSS (Practice suite solution)
 Experience working with low-income, marginalized, migrant and other priority groups in
the community on health and social service issues.
 Demonstrated knowledge of community resources, health promotion and social
determinants of health and public health policies and their impact on marginalized and
racialized communities.
 Experience in coordinating and facilitating health promotion programs.
 Experience in developing and managing community-based projects.
 Ability to manage different activities and projects.
 Excellent interpersonal and communication skills (written and verbal) and group
facilitation skills.
 Experience in project implementation and evaluation.
 Ability to multi-task, work in a fast paced environment, and meet deadlines.
 Well-organized with the ability to prioritize work assignments and maintain confidentiality.
 Proficiency in the use of computer and various software applications including Microsoft
Word, Excel, PowerPoint, and Outlook.
 Experience managing events and volunteers.
 Ability to work a flexible schedule based on the need of the program.
Salary: $ 64,046 to $ 77,161 (depending on experience) plus HOOPP and Benefits
Hours: 35 per week
