Rexdale CHC’s Diabetes Education Program offers FREE services to individuals, groups, caregivers, and employers to assist people living with diabetes to better manage the disease and prevent complications that can limit one’s independence. We also counsel people with pre-diabetes or at risk of developing type 2 diabetes to prevent/delay the onset of diabetes.

People diagnosed with Type 2 diabetes, prediabetes and those at risk of developing type 2 diabetes who live in the northwest corner of the GTA. Refugees and newcomers are welcome. Doctor’s referrals and self-referrals accepted. Health insurance is not required.
